Holiday Toy Drive Nomination Form

Families Building Community, the Elmhurst Community Partnership Project is looking for children in the Queens community who could benefit from holiday toys. Do you know of a family who could benefit from a little holiday cheer? If so, please complete nomination form and email it to (JavaScript must be enabled to view this email address), or fax it to us at 718-507-4247. If you would like more information about the holiday toy drive/party, feel free to give us a call or send us an email.

Please submit the completed form to Families Building Community via email by December 4th, 2010 or by fax. The maximum amount of children we can assist with is five per agency/organization. Agency will receive a confirmation sheet upon acceptance. The holiday toy party will take place on Friday, December 17th, 2010, from 4 pm to 8 pm. No more than two parents per child will be able to attend party.
